Free Personal Training Agreement Template (PDF)
A personal training agreement is a contract between a personal trainer and a client. It sets the sessions or package, price, scheduling and late-cancellation rules, and health disclosure, and includes the client's assumption of risk and a release of liability.

When to use it
Use it before a new client's first session, whether you train in a gym, at the client's home, outdoors, or online. Combining the service terms and the waiver in one document means a new client signs once on their phone and you have both in writing.
What's typically included
- Trainer, client, and date
- Sessions or package and location
- A clear statement that the trainer doesn't give medical advice
- Price, package expiration, and refund policy
- Scheduling, late cancellations, and no-shows
- Health disclosure and doctor's clearance
- Assumption of risk
- Release of liability, excluding gross negligence
- Parent or guardian signature for minors
- Trainer's certifications and insurance
- Ending the agreement
- Signature blocks for trainer and client
How to fill it in
- Decide your refund policy for unused prepaid sessions, and check whether your state regulates prepaid fitness or health club contracts.
- Ask every client about medical conditions, injuries, and medications before the first session, and require a doctor's clearance when in doubt.
- Keep the release as written: courts generally won't enforce a release of gross negligence, and a release that overreaches can be thrown out entirely in some states.
- For a client under 18, have a parent or guardian sign as the client.

Frequently asked questions
Do personal trainers need clients to sign a waiver?
It's strongly recommended, and many insurers require it. A waiver with an assumption of risk shows the client understood the risks of exercise. It doesn't replace liability insurance.
Are fitness liability waivers enforceable?
Often, for ordinary risks and ordinary negligence, if they're clear and signed voluntarily. They generally don't protect against gross negligence or intentional misconduct, and a few states limit them further. This template is drafted with those limits in mind.
What should a personal training cancellation policy say?
Most trainers count a session as used if the client cancels with less than 24 hours' notice or doesn't show. Whatever you choose, put it in writing and apply it consistently.
Can a personal training waiver be signed electronically?
Yes. Under the U.S. ESIGN Act and state UETA laws, an e-signed waiver is as valid as a paper one, and DocuAgree keeps a certificate showing when and how the client signed.
